Your doctor knows when you are ready to be discharged from the Hospital.While the Hospital
endeavours to provide speedy and cost effective and health care in the most efficient manner
it knows how to; any presure tactics,employed by the patient/relatives for premature discharge
are strongly resented by the doctor in-charge since he may be held liable for violation of Discharge
guidelines by the Hospital.premature discharge is tantamount to non-compilance on the part of
the patient,which is detrimental to a healthy doctor-patient relationship.

Patients leaving the Hospital against medical adviceare required to sign certain documents.
They may not be welcome as patients again.

Normal check-out time is 12:00 noon.

Hospital discharges are not made on Fridays and Holidays,unless prior arrangements are made
with the doctor in-charge during the normal working hours.

Prior to discharge,your doctor will advise you regarding the activity,diet,medications,if any,
and subsequent follow-up visits to the Hospital.please follow the instructions carefully. please
do not forget to bring your medical reports during follow-up visits to the Hospital.

At the time of Hospital discharge, you will receive a copy of Patient Feedback Form, which you may
complete your own convenience. Do not forget to pass them back as your opinions and suggestions are valued highly.

Hospital Property at the time of discharge must be handed over to the Nurse in-charge.

In the event of death, Release of Dead body Form must be completed before the body can
be handed over to the relatives
